RadioShack sells a cable that splits a 3.5mm stereo jack into two RCA phono plugs. They also sell a RCA-1/4" plug adaptor. So with one of the former and two of the latter, you have what you need.
They also sell a 3.5mm stereo jack to two mono 3.5mm splitter which you could use with two mono 3.5mm to 1/4" adaptors.
