HERE is one that can be ordered in several different configurations including what you need...see description. However, I don't think you'll find your configuration in anything of higher quality than this, whereas if you go the adapter route you can geek out on the RCA interconnects as much as you like.
Someone like Blue Jeans cable may build you a cable. Might be worth a call to them.
Thanks. I'm a Sam Tellig fan, too, by the way. I checked out the ZZ sounds site, but the cable has got mic blogs at the "Y" ends and I need male RCS plugs... I will try Blue Jeans cables.
